Option 1
Type the email address that you used when you enrolled, and instructions will be emailed to that address:
If you do not receive the email within 30 minutes, either the email address supplied had a typo, or has changed since you enrolled, the email sent out was filtered as spam by your email program, or for some other reason was not able to be delivered. In that case, you can try this option again if you like, or option 2 below.
